LOSING 13 KG IN 3 MONTHS: MORE THAN JUST A NUMBER ON THE SCALE

Overweight and obesity affect more than appearance. They also raise the risk of metabolic conditions such as diabetes, dyslipidemia, and fatty liver disease.
That is why obesity treatment is not simply about reducing the number of kilograms on the scale. It also aims to improve metabolic health and control accompanying conditions. A key challenge in this process is to lose primarily fat mass while minimizing muscle loss as much as possible.
The journey of a 50-year-old female client who lost 13 kg in 3 months at CarePlus is an example of how important it is to monitor body composition and adjust the treatment plan throughout the weight loss process.
1️⃣ The start of the weight loss journey
The 50-year-old female client came to CarePlus for a nutrition consultation, weighing 83.3 kg with a BMI of 34.7 kg/m². Following examination, she was found to have type 2 diabetes, hypertriglyceridemia, hyperuricemia, and severe fatty liver disease at the same time.
She received coordinated treatment from a Nutrition Specialist and an Endocrinologist, with the following plan:
▪ An energy-restricted diet of approximately 1,300 kcal/day, individualized according to her nutritional status and body composition.
▪ Gradually building an exercise habit, starting with a 15-minute walk every day.
▪ Combined treatment with semaglutide and medications to control her metabolic conditions, as prescribed by the Endocrinologist.
2️⃣ After 4 weeks: rapid weight loss, but not yet optimal
📊 After the first 4 weeks, the client had lost 5.8 kg. However, her InBody body composition analysis showed:
▪ Fat mass decreased by 2.3 kg.
▪ Muscle mass decreased by 2.0 kg.
▪ Body fat percentage rose from 47% to 48.5%.
Although her weight dropped significantly, the result was not truly optimal because she had also lost a considerable amount of muscle. This is a concern, as muscle loss can affect muscle strength, physical function, and the ability to maintain weight in the long term.
Fast weight loss is not necessarily effective weight loss if it is judged only by the number of kilograms lost.
Based on this result, the Nutrition Specialist adjusted the protein content of her diet and added resistance exercises to limit muscle loss in the next phase.
3️⃣ After 3 months: 13 kg lost and improved metabolic health
The client continued her diet and added resistance training twice a week, using weights and resistance bands.
💪 After 3 months of treatment, the results were as follows:
▪ Weight loss of 13 kg, equivalent to 15.6% of her initial body weight.
▪ Fat mass decreased by 9.2 kg, while muscle mass decreased by a total of 2.4 kg.
▪ HbA1c fell from 8.02% to 6.23%.
▪ Triglyceride and uric acid levels returned to normal limits.
Notably, muscle mass dropped by 2.0 kg during the first 4 weeks, while over the following 2 months, only a further 0.4 kg was lost. These results show that the trend in body composition improved after the plan was adjusted, although continued monitoring is still needed.
Beyond weight loss, the client also experienced positive improvements in metabolic health, particularly in blood glucose and triglyceride control.
💚 Weight loss is a journey, not just a destination
According to CarePlus doctors, what matters in obesity treatment is not only how many kilograms are lost, but where those kilograms come from.
Therefore, in addition to monitoring body weight, assessing body composition, adjusting dietary protein, and incorporating resistance training are important factors in helping to achieve the goal of reducing fat while limiting muscle loss.
The client’s treatment journey is still ongoing. In the coming phase, she will continue to be monitored and have her plan adjusted to work toward her target weight, keep her accompanying conditions well controlled, and consider reducing medication when appropriate for her health condition.
Because weight loss is not only about improving appearance, but also about improving health. And lasting success lies not only in reaching a desired weight, but also in the ability to maintain those results through a healthy, sustainable lifestyle.
