A herniated disc occurs when the soft nucleus pulposus of an intervertebral disc pushes out through a tear in the annulus fibrosus and compresses the spinal canal or nerve roots. The most commonly affected areas are the cervical spine (C3–C4, C5–C6) and the lumbar spine (L4–L5, L5–S1).
